After an hour, Leonardo Spinazzola gave Jose Mourinho's team the lead after falling behind 1-0 from the first game.
The visitors appeared to be going through after Igor Paixao's late goal, but Pablo Dybala's goal in Rome's 89th minute forced extra time.
Then, Lorenzo Pellegrini and Stephan El Shaarawy made it 4-1, and Roma triumphed 4-2 overall.
In the Europa Conference League final from the previous year, Roma also defeated Feyenoord.
Their semi-final opponent is Bayer Leverkusen, who easily defeated Union Saint-Gilloise 4-1 in the second leg of their match to win 5-2 overall.
