The error involved the disallowance of a Luis Diaz goal for offside, which VAR failed to overturn despite clear evidence to the contrary.

The governing body for referees, PGMOL, has acknowledged the error, describing it as a "significant human error." VAR officials Darren England and Dan Cook were taken off duty for the rest of the weekend as a result.
Liverpool has found the PGMOL's explanation unacceptable and has called for a review of the incident with full transparency. They argue that the disallowed goal had a significant impact on the match's outcome.
The controversy surrounding the disallowed goal was heightened by the fact that Liverpool was already down to ten men following Curtis Jones' controversial red card, which also involved VAR intervention.
